Make a Referral

Tahwil House Inc accepts referrals from community organizations, social service agencies, healthcare providers, correctional reentry programs, employment agencies, and other community partners.
 

Our mission is to provide supportive services, housing assistance, workforce development resources, and community-based programs that help individuals successfully transition toward stability and self-sufficiency.

Who Can Refer?

  • Social Workers

  • Case Managers

  • Healthcare Providers

  • Community Organizations

  • Reentry Programs

  • Employment Agencies

  • Family Members (where applicable)

Referral Process

  • Complete the Referral Form.

  • Submit all required information.

  • Our team will review the referral.

  • A staff member will contact the referred individual or referring agency.

  • Eligibility and program availability will be determined.
